A hospital visit costs a daily-wage household more than the consultation fee. It costs the fare there and back, and the day of work that is not done. For a condition that is uncomfortable but not yet urgent, that arithmetic usually ends in waiting.
Waiting is what turns a treatable problem into a serious one. By the time a delayed condition becomes urgent enough to justify the trip, it often needs more care than it would have a month earlier.
Holding consultations inside the neighbourhood removes both costs. People come in the gaps in their day. They bring family members who would not have made a separate trip. Conditions get seen earlier, at the stage where a straightforward course of treatment still works.
The limits matter as much as the reach. Community-level care is the right setting for consultation, diagnosis and common treatment. Anything beyond that needs a hospital, and our job in those cases is to recognise it quickly and help the family get there.
